How can I schedule an appointment?

Appointments can be made online through our website or through texting or calling. Please note calls made on days the office is closed may not be answered immediately and leaving a voicemail or text is helpful.

Do you offer tele-visits?

Yes! Sam is currently licensed in Arizona and Iowa. Tele-visits will take place over the phone with the option for video conferencing if desired. Schedule now!

What type of payment do you accept?

We accept cash, credit card, Venmo, HSA, and Zelle.

Do I have to purchase something at my first visit?

Absolutely not. Regardless of what you decide, all patients will receive a consultation with recommendations, handouts, and information about the medicines offered. The decision to purchase a medicine can be made at any time during or even days after the consultation.

Can this office be used as my primary care provider?

No, all patients engaging in pharmacological weight loss therapy are expected to have a primary care provider (PCP) that they regularly follow with. The purpose of this office is to assist with weight loss.

Am I locked into a subscription with this medicine?

You will never be locked into any payment plans or subscriptions. Start, stop, or restart your therapy at any time without commitment or pressure. Engage in a weight loss journey that works for your schedule and financial needs.

What if I need to cancel or reschedule my appointment?

Please text, call, or email our office if any changes or cancellations need to be made to your appointment.

How long are visits?

Initial visits are generally 20-30 minutes in length to ensure your needs are addressed and you are not being rushed. Subsequent visits for established patients are 15 minutes.
